With 16 teams, double elimination takes 30 matches (31 if the grand final is replayed): 4 rounds in the winners bracket and 6 in the losers bracket.

How to use it

Write the teams on the first-round lines. The small numbers are the seeds: put your strongest team on 1, the next on 2, and so on, so the favorites only meet at the end.
After each match, write the winner on the next line.
